Terms and Conditions

VitalSigns House Calls Inc.
Effective Date: June 17, 2026

These Terms and Conditions ('Terms') govern your use of the VitalSigns House Calls Inc. website,
patient portal, and healthcare services. By accessing our website or engaging our clinical services,
you agree to be bound by these Terms. If you do not agree, please discontinue use of our website
and services.

 

1. Description of Services
VitalSigns House Calls Inc. provides in-home primary care, chronic care management (CCM), remote
patient monitoring (RPM), behavioral health integration (BHI), transitional care management (TCM),
annual wellness visits (AWV), advance care planning (ACP), and related clinical services to eligible
patients. Services are provided by licensed, credentialed nurse practitioners and supporting clinical
staff in accordance with Alabama state law and applicable federal regulations.

2. Patient Eligibility and Enrollment
• Services are available to individuals residing in our designated service area in and around Mobile,
Alabama.
• Enrollment requires completion of intake documentation, signed consent forms, and verification
of insurance coverage.
• Medicare and Medicare Advantage beneficiaries must meet program-specific eligibility criteria for
each service type.
• VitalSigns reserves the right to decline or discontinue services in cases of patient safety
concerns, non-compliance, or geographic limitations.

3. Appointments and Scheduling
House call appointments are scheduled in advance. We require a minimum 24-hour notice for
cancellations. Repeated no-shows or late cancellations may result in a scheduling fee or discharge
from the practice at VitalSigns' discretion. Our standard office hours are Monday through Thursday;
the office is closed on Fridays.

4. Payment and Billing
• VitalSigns bills Medicare, Medicare Advantage, and other accepted insurance plans on your behalf
for covered services.
• Patients are responsible for applicable copayments, deductibles, and coinsurance as determined
by their insurance plan.
• Non-covered or elective services, or services to uninsured patients, will be billed directly at
published rates.
• Billing disputes must be submitted in writing within 60 days of the statement date.
• Unpaid balances may be referred to a collection agency after 90 days without a payment
arrangement.

5. Telehealth and Remote Services
Certain chronic care management, remote patient monitoring, and care coordination services are
delivered by telephone or secure electronic communication. You agree to maintain accurate contact
information and to participate in scheduled remote check-ins as part of your care plan. Audio or
visual recordings of telehealth encounters are not permitted without mutual written consent.

6. Patient Responsibilities
By receiving services from VitalSigns, you agree to:
• Provide accurate and complete medical history, medication lists, and insurance information.
• Notify VitalSigns of any changes in your health status, medications, or insurance coverage.
• Comply with your agreed care plan and treatment recommendations to the extent possible.
• Treat VitalSigns staff, clinicians, and contractors with respect and in a non-threatening manner.
• Secure a safe and accessible home environment for house call visits.


7. Intellectual Property
All content on the VitalSigns website — including text, graphics, logos, images, care protocols, and
educational materials — is the property of VitalSigns House Calls Inc. and is protected by applicable
copyright and trademark laws. You may not reproduce, distribute, modify, or create derivative works
without our prior written consent.


8. Disclaimer of Warranties
Our website and non-clinical digital content are provided 'as is' without warranties of any kind,
express or implied. VitalSigns does not warrant that website content is error-free, up-to-date, or
suitable for any particular purpose. Website content does not constitute medical advice and should
not substitute for direct clinical consultation with a licensed provider.


9. Limitation of Liability

To the fullest extent permitted by applicable law, VitalSigns House Calls Inc., its officers, employees,
and affiliates shall not be liable for indirect, incidental, special, consequential, or punitive damages
arising from your use of our website or services. Our total liability for any claim shall not exceed the
amount paid by you for the specific service giving rise to the claim in the three months preceding the
claim.


10. Governing Law and Dispute Resolution
These Terms are governed by the laws of the State of alabama. Any disputes shall first be subject to
good-faith negotiation. If unresolved, disputes shall be submitted to binding arbitration in Mobile,
Alabama under the rules of the American Arbitration Association, except that either party may seek
injunctive relief in a court of competent jurisdiction.


11. Non-Discrimination
VitalSigns House Calls Inc. does not discriminate on the basis of race, color, national origin, age,
disability, or sex in accordance with applicable federal and state civil rights laws, including Section
1557 of the Affordable Care Act. Auxiliary aids, language services, and reasonable accommodations
are available upon request.


12. Modifications
VitalSigns reserves the right to update these Terms at any time. Updated Terms will be posted on our
website with a revised effective date. Continued use of our services constitutes acceptance of any
modifications.
