Melanie Tims, MSN, CRNP, FMP-C

Founder & Lead Clinician

Melanie Tims, MSN, CRNP, FMP-C is the founder and lead clinician of VitalSigns House Calls. A board-certified Adult Nurse Practitioner with advanced training in Functional Medicine, she has spent the last eleven years providing high-quality, compassionate in-home geriatric care across Southern Alabama.

​

Her professional path began as an Educator at Bishop State Community College, followed by Oncology Nursing, Multi-Specialty Pharmaceutical Research Coordination & Investigation, and ultimately Advanced Practice in home-based care.

​

The daughter of missionaries, Melanie spent much of her childhood in Southeast Asia — an experience that instilled in her a lifelong love of travel, a deep appreciation for family and friends, and an unwavering desire to serve.

​

For Melanie, VitalSigns House Calls and its sister organization, Juniper Tree, are the places where her professional goals and personal convictions finally converge. Together with her founding team, she hopes to create a culture of care where faith is quietly lived out through excellence in medicine and a genuine desire to be the hands and feet of Jesus, offering patients not only clinical skill, but the simple, heartfelt support that makes healing feel possible.

Dr. Jonathan Dean, MD

Medical Director & Collaborating Physician

Dr. Dean is board certified in Family Medicine. Since completing his residency in 2012, he has provided comprehensive care for families across all stages of life — from newborns to end-of-life care.

​

A native of Mobile, Alabama, Dr. Dean attended Harding University in Arkansas for his undergraduate studies. He earned his medical degree from the University of Alabama at Birmingham (UAB) School of Medicine, graduating in the top 10% of his class. He then completed his Family Medicine residency at UAB Huntsville, where he received multiple awards for excellence.

​​​​Outside of medicine, Dr. Dean and his wife, Monica, cherish time with their children. They also enjoy traveling and serving together through international ministries around the world.

Laura Dragotta, RN

Care Coordinator

Laura Dragotta is a Registered Nurse and founding Care Coordinator at VitalSigns House Calls Inc. With a strong background in Pharmaceutical Research, Case Management and floor nursing, Laura brings extensive clinical knowledge, meticulous attention to detail, and a calm, steady presence to every patient interaction. Her experience across diverse healthcare settings allows her to anticipate needs and provide thoughtful, thorough support during both in-home visits and remote care coordination.​

She has a special love for all creatures, great and small, and carries that same gentle, nurturing spirit into her work, making patients feel truly seen and cared for.

Ebonee Graham, MA

Care Coordinator

Ebonee Graham is a founding Care Coordinator at VitalSigns House Calls Inc.

She lights up every room she enters with her open nature, warmth, and loving spirit, immediately putting patients at ease. With extensive experience in Pulmonology, Pharmaceutical Research and caring for patients across all age spectrums, Ebonee brings both strong clinical expertise and a genuine human connection to her role. Known for her reliability and heartfelt commitment to others, she creates a supportive and positive experience for every patient she serves.

Our Partnership with

Juniper Tree

Juniper Tree’s name comes from a quiet moment in Scripture, where the prophet Elijah—exhausted and overwhelmed—found rest beneath a juniper tree. In his weariness, an angel came to care for him with food, water, and gentle encouragement to keep going. That picture is at the heart of Juniper Tree: in seasons of deep strain, people are seen, supported, and strengthened, often through the caring actions of others. Real healing can begin with something simple—rest, nourishment, and the reassurance that we are not alone.

During her years caring for patients in their homes, Melanie Tims saw how 
quickly “small” issues could spiral when

people were living with chronic illness, limited resources, or very little support. A missed medication, a broken appliance, a cut-off utility, or a lack of transportation could undo months of progress and send someone back into crisis. Those experiences led to a two-part response: VitalSigns House Calls and Juniper Tree.

VitalSigns House Calls is shaping into a home-centered care and coordination service that wraps support around the patient where they live. Instead of relying only on office visits or short-term transitions after a hospital stay,
VitalSigns combines in-person home visits with ongoing remote coordination to notice small changes early, respond quickly, and help prevent avoidable emergencies or hospitalizations. Many of the individuals served are also connected with community agencies and support services, which makes it easier to see when practical needs are putting someone’s health and stability at risk.

Juniper Tree was created as the charitable arm that can step in when those practical needs become barriers. While VitalSigns focuses on medical care and coordination, Juniper Tree focuses on targeted financial help—things like medication co-pays, food in an emergency, utility bills, transportation, or procedure-related costs—so that a gap in resources does not become a major setback. Juniper Tree’s reach is not limited to VitalSigns patients; churches, community organizations, senior communities, healthcare  
providers, and concerned individuals can all refer someone who appears to be in need.

Every request is reviewed using Juniper Tree’s own eligibility guidelines, documentation requirements, and funding limits, and assistance is provided only as resources allow. Being a VitalSigns patient is not required to apply, and receiving care from VitalSigns does not guarantee approval of assistance. Both organizations share a single goal: helping people remain safer, more stable, and better supported in the place they call home.

In this early season, Juniper Tree is sustained through limited seed funding provided by its founder, with the long term plan for VitalSigns to contribute from its ongoing work, alongside support from churches, local businesses, and community partners. As those roots deepen in the community, Juniper Tree’s capacity to respond will grow, allowing more people to receive the kind of timely, practical help that can change the trajectory of their health and quality of life.

Practical Support When Needed

Juniper Tree helps patients with real-life challenges like medication costs, medical procedure and service copays, food insecurity, utility insecurity and transportation insecurity, ensuring care goes beyond the visit and daily life feels safer and more manageable.

Patients can join free classes and access scholarship programs designed to build skills in managing chronic conditions, wellness, and long-term health — giving knowledge, confidence, and independence.
