Find answers to the most common questions about receiving expert medical care in the comfort of your home.
Do I need a referral to receive care?
No referral is required. Many patients come to us directly after a hospital stay, through word-of-mouth, or because getting to a doctor’s office has become difficult. We also work closely with physicians and hospitals who refer their homebound patients to us.
Will this replace my regular doctor?
Not at all. We partner with your existing primary care physician. We send detailed notes after every visit so your doctor stays fully informed, and we happily transition you back to office care as soon as you’re able.
How long do visits last?
New patient visits are scheduled for 90–120 minutes so we have time to listen, perform diagnostics, and create a complete plan. Follow-up visits are 45–60 minutes. You’ll never feel rushed.
What if I have an urgent need?
During business hours we offer same-day or next-day in-home response for issues that don’t require the ER. Our Chronic Care Management program also provides weekly check-ins so small problems are caught early.
Are diagnostics done in my home?
Yes. We bring everything needed — EKG, spirometry, labs, hearing/vision screening, and more — so you receive a complete evaluation without leaving your living room.
How does Chronic Care Management work?
Once enrolled, our dedicated care coordinators check in with you weekly to review medications and coordinate services. Every plan receives direct oversight from our Nurse Practitioners, providing a proactive, team-based approach to prevent complications.
Is there any cost to me?
Most services are covered by Medicare and private insurance. For those with financial hardship, our partner Juniper Tree offers scholarship assistance on a case-by-case basis.
