For homebound patients or those who find office visits difficult due to
mobility, transportation, or other barriers, we serve as your in-home primary
care provider while maintaining close communication with your established
physician.

We provide timely in-home visits following hospital and skilled nursing

team performs medication reconciliation, comprehensive assessments, and
creates a clear care plan.
Proactive monthly support from people you know, who actually know you.
For patients with two or more chronic conditions, we provide structured monthly care coordination, education, and weekly check-ins from a local team that knows your name and your story.

We use simple, FDA-cleared home devices to track key vital signs and catch changes before they become emergencies.
Insurance Co-pays & Financial Assistance
Many Medicare Advantage plans offer extra benefits that patients often don’t know about or fully use, such as monthly over-the-counter allowances (commonly $20–$150+), fitness memberships, meal delivery, transportation, and more. During your visits or monthly check-ins, our team can provide a simple checklist of common extra benefits to help ensure you are maximizing your benefits! Most services provided by VitalSigns House Calls are covered by Medicare and Medicare Advantage plans. Many plans cover these services with little or no out-of-pocket cost. Patient responsibility is typically low, and if any co-pay creates a hardship, scholarships through our sister nonprofit, Juniper Tree, are available so that financial need never prevents someone from receiving care.
